How to Convert Passive to Active Voice

"The report was written by the team."
Active
"The team wrote the report."
Passive
"Mistakes were made."

"We made mistakes." or "Management made mistakes."
Formula: Find the "to be" verb + past participle, identify who is doing the action (often in a "by" phrase), then rewrite: [actor] + [active verb] + [object].
When passive is OK: Scientific writing, when the actor is unknown, or when the object is more important than the actor.
